About the Role
Join the Rehabilitation in the Home team and help patients rebuild strength, confidence and independence. You will deliver evidence based exercise physiology care, support recovery after complex health events and work closely with a multidisciplinary team to achieve safe discharge and hospital avoidance.
• Assess patient needs and tailor exercise therapy plans
• Deliver evidence based programs in home settings
• Collaborate with nurses, doctors and allied health teams
• Support safe discharge and hospital avoidance strategies
• Guide assistants and students and contribute to service improvement
About You
You are a compassionate Exercise Physiologist who values evidence based care and teamwork. With strong clinical judgement and communication skills, you support people with complex rehabilitation needs to safely rebuild strength, independence and confidence.
• Accredited Exercise Physiologist with ESSA recognised degree
• Apply evidence based rehabilitation and clinical judgement
• Communicate clearly and maintain accurate clinical records
• Work collaboratively with multidisciplinary care teams
• Use digital systems such as EPAS for clinical documentation
