Contract: Full-time, permanent
Working Hours: 37.5 hours per week, including participation in a weekend rota
Salary: £47,810 - £54,710 per annum, full-time equivalent

About the Role
As a Community Clinical Nurse or Paramedic Specialist, you’ll provide advanced specialist palliative care, support and advice to patients, carers and external professionals. Working autonomously within our community team, you’ll deliver high‑quality, person‑centred care — whether that’s in a patient’s home, via telephone, or within our outpatient clinic.
What You’ll Be Doing
Delivering advanced palliative interventions in partnership with the wider multidisciplinary team
Providing specialist advice to colleagues internally and externally (GPs, district nurses, care homes, social care)
Undertaking holistic assessments and devising personalised care plans
Supporting and mentoring junior colleagues and students
Triaging referrals and contributing to caseload management and daily team prioritisation
Offering expert guidance on symptom control, complex decision‑making and ethical issues
Using advanced communication skills to support patients and families through psycho‑social challenges
Participating in clinical supervision, audits, working groups and service development
Maintaining accurate, timely documentation on SystmOne
Keeping your practice aligned with national policy, professional codes and hospice values
What You’ll Bring
Registered Nurse (NMC) or Paramedic Practitioner (HCPC) with recent CPD
Experience delivering specialist palliative care and working autonomously in complex clinical situations
Advanced communication, assessment and clinical decision‑making skills
Strong understanding of palliative and end‑of‑life care, symptom management and community support pathways
Confidence using IT systems (ideally Office 365 and SystmOne)
Emotional resilience, compassion and the ability to work flexibly in varied community settings

A driving licence and the ability to travel within the community is essential for this role.
