About the Role:
The Community Health Paramedical Aide provides high-quality, home-based care to clients under the Commonwealth Home Support Programme (CHSP). Working within a wellness and reablement framework, you will support frail older people aged 65 years and over (50 years and over for Aboriginal and Torres Strait Islander people) to remain living safely and independently at home and in the community.
You will work collaboratively with clients, families, carers and advocates to develop and implement individualised care plans. Using your professional judgement, you will deliver flexible, responsive and culturally appropriate services aligned with CHSP guidelines, ensuring client choice, dignity and high-quality outcomes are always at the centre of care.
